It is recommended for you to arrive 15-20 minutes before your first appointment to give you enough time to fill out the necessary paperwork needed.
Please bring the following to Rodriguez-Aquino MD PA:
Picture ID: Please bring a form of picture ID with you. State ID or Driver’s License is preferred.
Insurance Card: If you have insurance please bring your insurance card to the clinic. Our receptionist will check what your insurance can cover.
Form of payment: If you are private pay or pay a part of the expenses please bring cash or a debit card.
Medications: Please bring the medication you are currently taking; we recommend for the medication to be in their original containers
